Thanks for coming to ClearChoice. We’re excited to meet you! We have launched changes to allow for social distancing in order to keep you safe.

We are asking that when you come to the office that you follow these guidelines:
1. Please fill out our welcome packet prior to entering our office: https://bit.ly/2WYiKng. After you complete your form, please print it out and bring it with you. If you can not do this then we will provide you with a printed copy in our office.
2. When you arrive at our center, please text us at (440) 740-0400 after you park your car. Once we know that you have arrived, we will text you back and let you know when we’re ready for you to enter the building.
3. We ask that you wear a mask, bandanna, or scarf at all times when you’re in the office.
4. Upon arrival into our building, we will be taking your temperature. If you are running a fever, we will ask you to reschedule.
